Designed for school populations through Grade 12. Items 13 and 14 explicitly reference school context ('out of school', 'courses I should take in school') — adult respondents may find them odd. Subscale alphas in the original sample were modest (Concern .62, Curiosity .74, Confidence .78, Consultation .69); total readiness alpha was .86. The paper does not reprint a participant-facing instruction stem; the one shown here is a generic agree/disagree prompt. The 24-item Counseling Form has no published cut-off bands; the separate 10-item Screening Form does. Authors made Form C free for users; cite Crites & Savickas authorship.
